About the Smart Referral Network
The Smart Referral Network (SRN) is a secure, regional referral platform that helps partner agencies connect individuals and families to community services more efficiently. SRN supports closed-loop referrals, coordinated service delivery, and shared visibility into referral outcomes across participating counties.
What SRN Does
SRN helps agencies send, receive, and track referrals in one shared system. Case managers and frontline staff can search for programs, identify services that may fit a client’s needs, send referrals electronically, and update referral statuses as services are reviewed or provided.
Why It Matters
When residents need support, they often interact with more than one organization. SRN gives partner agencies a shared referral workflow so clients do not have to start from scratch each time they seek help. The platform is designed to strengthen coordination, reduce referral gaps, and make it easier for agencies to follow up on whether a referral led to services.
Regional Partnership
SRN is a regional solution supporting collaboration among participating United Ways, community-based organizations, public agencies, and service providers. The regional page helps users find the right county resources, access training materials, and connect with support for onboarding or technical questions.
How the Network Works
Find services: Users search for programs and services that match a client’s needs, location, and eligibility.
Send referrals: Partner agencies send referrals electronically to participating programs.
Track progress: Agencies update referral statuses so senders can see whether referrals are pending, accepted, enrolled, declined, or closed.
Support privacy: Client information is shared based on consent, permissions, and each agency’s role in serving the client.
For Partner Agencies
Participating agencies play an active role in keeping the network effective. Partners are expected to maintain accurate program information, respond to referrals in a timely manner, protect client privacy, and use SRN training resources to support staff who send or receive referrals.
